At Children’s Health Care we realize the impact that social determinants of health (SDOH) have on your family’s health. SDOH factors that impact education, housing, living environment, income, stress, social inclusion, job or work insecurity, social support, food and transportation.

Our MassHealth patients who indicate any need pertaining to SDOH will be referred to one of two staff members. This staff is provided to our practice through our partnership with MassHealth (Steward Health Choice). Non-MassHealth patients can discuss local resources with their provider as well as Barbara Kneeland, our special care coordinator.

We also partner with Our Neighbor’s Table, a local service organization helping to alleviate food insecurity to residents of Amesbury, Boxford, Byfield, Georgetown, Groveland, Merrimac, Newburyport, Newbury, Salisbury, Merrimac, and South Hampton, NH. They offer fresh, nutritious groceries, community meals, and holiday groceries. With our partnership, we will help identify families in need and start the referral process. Our Neighbor’s Table also provides emergency food bags in our office for our families.
